Improve address domain model

As a business user I would like to capture more detailed address data of my customers.

TD:
add phone1, phone2
add email1, email2
add mobile1, mobile2
add custom1, custom2, custom3, custom4

add configuration for list of fields to be shown on the address form
add configuration for names for custom fields
adjust address format to be able to capture additional fields
